Eligibility |
Principal Employees - All employees upon hire who are at least 18 years old to less than 65 years old as of the effective date.
- When the Member's age exceeds the maximum permissible age, coverage will continue until the expiry date.

Married Principal Employees
- Start with a legal spouse less than 65 years old.
- Followed by children (eldest child, down to the youngest), 15 days to less than 21 years old, unmarried, and not gainfully employed.
- Extended Dependent - Parents of married employees should be less than 65 years old and not gainfully employed.

Single / Unmarried Principal Employees
- Parents who are less than 65 years old.
- Siblings (eldest down to the youngest), 15 days to less than 21 years old, unmarried and not gainfully employed.

Single Parent Principal Employees
- Children (eldest down to the youngest), 15 days to less than 21 years old, unmarried, and not gainfully employed.
- Parents who are less than 65 years old and not gainfully employed.

Unmarried Domestic Partner/Common-law partner and same-gender domestic partner less than 65 years old.
Guidelines:
- Common-law and Same-gender Domestic Partner may be enrolled as a dependent, subject to submission of proof of coverage and CENOMAR and/or Affidavit of Cohabitation, as endorsed by the Authorized HR personnel. Deletion/replacement of partner shall not be allowed in the middle of the contract year.
✓ Common-law and Same-gender Domestic Partner dependents must be at least 18 years to less than 65 years old. ✓ Both partners should be single, not legally married, or the domestic partner of anyone else. ✓ The enrolment of the dependent partner must be duly endorsed by HR.

Pre-existing conditions |
- Principal Employees - up to Maximum Benefit Limit (MBL)
- Existing Dependents - 2nd year: up to Maximum Benefit Limit (continuously covered for at least 12 months)
- New Dependents - up to PHP 10,000.00
